Transcript Request Guidelines and Form


 

Any outstanding financial obligations to Otis College of Art and Design must be met prior to the release of transcripts.

Fill out, sign and date the Transcript Request Form. (can be completed before you print it)
Be sure to include "Attention to" or department information in the address.
Lack of information will delay the process of your request for transcripts.

The fees for Official transcripts are as follows:
Regular Delivery: $5:00 per transcript (mailed in 8 business days)
Rush Delivery: $25.00 per transcript (processed in 72 hours for pickup)
Rush Delivered $25.00 per transcript in the U.S. 72 hours by overnight service
  to a street address only, Overnight Services do not deliver to P.O. Boxes
There is no charge for unofficial transcripts (5 business days)

Payments can be made by:

  • Visa and MasterCard card
  • Personal Checks and money orders payable to "Otis College"
  • Cash at the Cashier's window Monday - Friday 8:45 am to 4:45 pm.
