Immunization and Screening

You can satisfy these immunization requirements by uploading required documents to OwlCare Online, our online health portal starting May 1st, 2025. Forms emailed or faxed before May 1st will not be accepted.

All incoming Otis College students are required to complete the New Student Health Form, Tuberculosis Screening Questionnaire, and provide documentation of vaccination or immunity to certain infectious diseases. Students will have a hold on their account until all immunization requirements are fulfilled. For detailed information regarding required vaccinations, please review the Immunization Requirements below. If a student would like to fulfill their immunization requirements at the Student Health and Wellness Center (after the due date), they will incur a late immunization fee (please see below for more details).

Required Immunizations

    • MCV4 or MPSV4 (Meningococcal) - Required if living in the Residence Hall or Park West through Otis Housing

    • MMR (Measles/Mumps/Rubella) - Required 2 doses

    • TB Screening/Testing - Required

New and Incoming Students (Living in Otis Housing or Off-Campus)

  • Upload your completed "New Student Health Form” on OwlCare Online.
  • Upload a copy of the supporting documentation onto OwlCare Online (yellow card/ vaccination records, blood tests, or letters from your health care provider).
  • Complete required forms on OwlCare Online.

Returning students who are now living in Otis Housing (Residence Hall or Park West):

  • You are required to have the MCV4 or MPSV4 Vaccine and the COVID-19 Vaccine 
  • Upload a copy of the supporting documentation onto Owl Care Online (yellow card/ vaccination records, blood tests, or letters from your health care provider).
  • Complete required forms on OwlCare Online.

Completing Immunization Requirements with the SHWC

For students who do not have access to their primary care provider, or those having difficulty meeting the immunization requirement in their current location, these immunizations can be completed at the SHWC. These immunization requirements are free for students who are enrolled in the student health insurance starting August 15th, 2025.  Please note that new incoming students have access to complete their immunizations at the SHWC starting July 1st, 2025; however, students will pay for these vaccines out-of-pocket (please see below for a breakdown of cost).  In addition, please be aware that students who wish to fulfill their immunization requirements at the SHWC after the deadline will incur a late fee*.

*Any immunizations received at the SHWC after the deadline are subject to a $20 late immunization fee after July 1st, 2025 and increases to a $100 late immunization fee if submitted after August 1st, 2025

Immunization Costs:

  •  TB Testing: $20 (PPD Skin Test)
  • TB Blood Test: $60 (Quantiferon Gold)
  • Meningitis Vaccine $195
  • MMR Vaccine: $145 per vaccine (2 doses are required)
  • MMR titers: $25
