Student Health Insurance Plan
All degree seeking students enrolled in BFA/MFA programs are required to have health insurance.
All currently enrolled students are automatically enrolled in Otis College's Student Health Insurance Plan, unless a waiver is completed. Otis College's insurance plan is administered through CHP (Consolidated Health Plans). It is a full service plan that utilizes the Cigna PPO network.

ID Cards: ID cards must be printed from online. They are not mailed out.
Waiving the Otis College Student Health Insurance Plan
In order to save the cost of the health insurance plan, you MUST complete an online waiver by the deadline. Please find further information and instructions here.
Tax Information/1095B Forms
If you were a member of the Student Health Insurance Plan during the 2016-2017 plan year or the 2017-2018 plan year, CHP will be sending you a copy of your 1095B form for tax filing purposes. The 1095B forms are NOT available online. They will be sent hardcopy to the address that CHP has on file for you. Please make sure that CHP has your most updated address by emailing customer service at email@example.com.
If you have questions about receiving your 1095B form, please call NGL (the carrier responsible for sending out the form) at (800) 756-3702.
You may enroll eligible dependents onto the health insurance plan as long as you are covered. If you would like to do so, please click the link below. For MFA Graphic Design students, the Summer deadline to enroll dependents is 6/23/17. For BFA (undergraduate) and MFA students, the Fall deadline to enroll dependents is 9/15/17.
If you waived the school's insurance, then lost your outside coverage
If this applies to you, you are still obligated to have health insurance coverage while at Otis College.This most commonly occurs in the following scenarios.
- Students turns 26 and loses coverage under their parents
- Covered CA or Medi-Cal coverage is not renewed
- Parent loses coverage through work so student no longer has their coverage
If you lose health insurance coverage part way through the plan year, you have the option to get onto the school's health insurance plan for the rest of the plan year. Your coverage will be pro-rated so that you only pay for the months that you receive coverage. If you would like to get onto the school's insurance mid-year, or have questions about this policy, please contact the Insurance Coordinator, Catherine Kreamer, at firstname.lastname@example.org.
Leave of Absence & not re-enrolling
The school's insurance plan covers you each semester that you are enrolled. Should your matriculation continue into the next semester, your coverage will also continue. If you do not enroll, or take a leave of absence less than 30 days into the next semester, your insurance will be terminated (end) the last day of coverage for your last enrolled semester.
- Example 1: You are enrolled for Fall 2017, but do not enroll for Spring. Your coverage will end on the last day of Fall 2017 coverage.
- Example 2: You are enrolled for Fall 2017, and enroll for Spring 2018, but then take a leave of absence less than 30 days into the Spring semester. Your coverage be terminated back to the last day of Fall 2017 coverage.
If you take a leave of absence 31 or more days into the semester, your insurance will remain in place for the rest of that semester.
Please note: There is often lag time bewteen your coverage ending and the insurance company's system showing that your membership is terminated. Should you use your insurance during that time, you will later be responsible to pay back anything that the insurance paid for you during that time. It is requested that you do NOT attempt to use your insurance after the termination date.
